Health & Fitness Liability Waiver / Informed Consent Form
I, ………………………………………………have enrolled onto a fitness program offered through Aspen Fitness Inc. and I recognize that the program may involve challenging physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities.
I hereby affirm that I am in good physical condition and do not suffer from any known disability orcondition which would prevent or limit my participation in this exercise program.
I acknowledge that I am responsible for making my own assessments regarding all the fitness and nutritional information provided to me, and that where necessary, I shall consult a licensed medical/nutritional practitioner for clearance to participate in the said fitness program. I acknowledge that my enrollment and subsequent participation is purely voluntary and in no way mandated by Aspen Fitness Inc.
I fully understand that I may injure myself as a result of my enrollment and subsequent participation in this program and I hereby release Aspen Fitness Inc. and its agents from any liability now or in the future for conditions that I may obtain. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat stroke/exhaustion, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness that I may incur, including death. Skylight Soccer Complex, Rembrandt's and Aspen Fitness Inc. are in no way responsible for any personal injuries sustained and/or loss of any personal possessions.
Media/Photo Release & Waiver
I grant permission to Aspen Fitness Inc. and its agents, employees or assigns, the irrevocable right to use the photographs or video media which I may appear in and provided by me, for use in any Aspen Fitness Inc. publication such as advertising, direct mail, brochures, newsletters, magazines and to use such photographs in electronic versions of the same publications or on any of our affiliated websites or other electronic form of media. I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs.
I HEREBY AFFIRM THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.
